An exciting opportunity has arisen for two Band 6 YOS specialist nurse posts (22.5 hrs pw) working alongside the Youth Offending Service countywide team. You will be working directly with young people to identify their health needs and reduce their risk of offending. The YOS Specialist Nursing team provide comprehensive health assessments to young people aged 10-18 who offend or may be at risk of offending. All clinical appointments are delivered on an outreach basis and also involve making referrals to relevant mainstream and specialist health services and liaising with your NHS and council colleagues. You will have the opportunity to have regular contacts with young people in order to build their trust and to deliver an individual package of care. We are looking for an enthusiastic and motivated individual to join our friendly team. You will be a qualified SCPHN (Specialist Community Public Health Nurse) who has had significant experience in a role working directly with adolescents. The nature of work undertaken with young people addresses a wide range of health issues such as physical health, mental health and emotional well-being, access to services (GP, dentist etc), sexual health, substance use, and neurodisabilities (ASD, ADHD, communication etc). Permanent post 22.5 hours per week (Full time hours may also may be considered dependant on interest) Available posts are based in the mid and west of the county.
