Cumberland Hall Hospital is a 97 bed, licensed, acute psychiatric hospital located in beautiful Hopkinsville, KY. Cumberland Hall features individual units for youth, adults and active duty military, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average over 1500 patients receive care from our compassionate health care team each year at Cumberland Hall Hospital.

Qualifications POSITION SUMMARY: To provide quality case support and services to inpatient and/or partial-hospitalization as assigned.
Works with IP & OP therapists to facilitate after-care appointments that may include sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge.
Collects collateral and supporting information for treatment team review.
Other duties of support for both IP and OP programming
Collaboratively complete status with updates for patients.
Coordinates with external partners/referral sources
Assist in the treatment program and implementation of patient treatment plans under the direction of the therapist
Implement appropriate therapeutic activities and interventions within the therapeutic environment
Facilitates groups and uses therapeutic techniques to interact with patients.
Clerical/administrative support duties
Complete necessary documentation
Check in patients, searches, secure belongings.
Must be able to observe equine therapy (ie – no horse allergy)


QUALIFICATIONS:
Education/Experience: Education High School Diploma required
Bachelor’s degree preferred

Experience One to two years of mental health experience required
Inpatient experience preferred.

Certifications/Licensure HWC (Handle with Care) required. This training also offered upon hire.
BLS certification


Licensure/Certification: Current Driver’s License
HWC Certification (Can be obtained after hire)
BLS (Can be obtained after hire)


Knowledge and Skills: Knowledge of the discharge planning process
Knowledge of resources available in the communities we serve
Professional verbal and written skills needed for communicating the patient status and treatment needs to payers, families, professionals, paraprofessionals employees of the hospital, and members of the patient's community.
Ability to maintain professional records.
Ability to maintain patient confidentiality and high ethical standards
Ability to maintain professional relationships with patients, patient’s family members, staff and community personnel.
Knowledge of milieu management.
Skills in preparing and maintaining appropriate medical record documentation.
